Intracranial Hemorrhage: A Case Study

A patient was chosen with longstanding chronic problems that included atrial fibrillation, diabetes, and hypertension. Two of the longstanding chronic problems, atrial fibrillation and hypertension, were diagnosed in 1992. One longstanding chronic problem, diabetes, was diagnosed in 2005. The patient reports that the chronic problems were managed on an outpatient basis and he has never required hospitalization until now. The patient reports that he was under the care of two physicians to manage his chronic problems, a cardiologist and an internist. Since this diagnosis the patient has transitioned from the hospital emergency department in the initial local hospital to a receiving hospital in San Antonio. This transition required the utilization of air medical services. Once at the receiving hospital the patient was initially admitted to an intensive care unit and eventually was transitioned to a lower level of care in an inpatient nursing unit. The most recent transition was from the inpatient nursing unit to an inpatient acute rehabilitation unit. The patient has already begun to prepare for transition to home. During these transitions the patient has been under the care of multiple physicians, including an emergency room physician, hospitalist/internist, tele-neurologist, neurosurgeon, and most recently a physiatrist. The patient’s cardiologist was also notified of the admission and was consulted to contribute to the management of the patient’s care.
