As a pharmacist, I am participating in hospital rounds and I am consulted to provide my clinical judgment on a therapy that the physician is thinking of developing for the patient, since it was learned that the patient was non-adherent mainly because he did not know how to use well his insulin and he barely check his glucose levels. I communicate with the team and provide my advice; in addition, I offer to counsel the patient on complementary dietary changes that would go well with the treatment and to properly show the patient how to use the medical equipment. After the team has agreed upon each team member’s role and on the appropriate procedure to be taken towards the patient welfare I meet with the patient, counsel him/her on their insulin medication and how to properly use their needle for injection. In addition, I demonstrate how the patient can monitor his blood glucose at home using a glucometer. At the end, I will make sure the patient has understood the instructions by asking to explain and if applicable, demonstrate how will they use the medical equipment.
