N presented with anxiety, depressed mood, anger, sleep difficulty, difficulty concentrating, recurring thoughts of death, family related stress, academic stress, and financial concerns. He reported that he periodically experiences elevated mood. He stated that such mood usually lasts for approx. 3 days. According to him, during the time, he would be more talkative and have more energy. However, he noted that he usually continue to feel depressed even while experiencing elevated mood. He reported that his family especially his mother and sister depend on him. He stated that it is difficult to focus on his own needs. He noted that his family do not validate his needs.

N sought service at the Center because his insurance allows only 12 sessions

10 min. late. He was Ox4. His mood was depressed and anxious, his affect was congruent, and his speech was normal. He was dressed appropriately for the weather. He was forthcoming about his experiences and challenges. He came across as friendly. He answered my questions without apparent hesitation. He attributed his mood to his family dynamic and a lack of support from his family.

ACADEMIC FUNCTIONING (e.g., Educational history, Current functioning, Employment):
N reported that he is currently taking 3 classes (15 credits). His reported cumulative GPA is 8. He reported that he often feels busy and hard to focus on his school. He reported that he works at a beverage retail store.

SOCIAL SUPPORT (e.g., Current living situation, friends, recreation, hobbies):
N indicated that he does not have a significant support system out side of his family. He indicated that he interact with co-workers at his work. However, it is not clear that he finds them supportive or close.

He noted that he lives with his mother. He reported that he has one older brother and one younger sister. According to him, he, his mother and sister moved to WA from TX in 2014 after his father stopped paying mortgage on the house they lived. He stated that he had to leave many items that had sentimental values for him. They moved to WA because his older brother was already here in WA. He reported that he does not get along with his father. He stated that he and his brother do not are not close. He stated that his brother and father has a similar personality. He reported that his father lives in TX. Moreover, he reported that his mother is anxious and worry about many things. He characterized her as "paranoid". He also stated that his sister has difficulty coping with stress in general that she avoids interacting with family when they have needs/demands. In addition, he noted that he feels that the family perceive him as the organizer of the family. Thus, he stated that his family ask him for help often.

N reported that he has been seeing a therapist in the community since late 2016. He stated that his insurance benefit is running out.
