On 3/29/17 I spoke with Ms. Messing. She reports that her neck remains stiff. She is still in physical therapy but was told in about a week she would be done. They would then give her a home exercise program to continue independently. She reports she is still swaying and her balance is still off. I asked if an ENT appointment had been made or if she had any appointments coming up and she said no.

On 4/3/17 I was contacted by Gail, Ms. Messing’s daughter. She reports her mother has an appointment today. They are requesting a open claim letter to be faxed to Dr. Ramsey PCP’s office. She reported she did not know about the appointment either until this day. Gail is unable to attend due to short notice and asked if I could attend. I faxed the letter to Dr.

Ms. Messing arrived without a walker. She holds her husband’s arm when she is walking. She reports that she still has the swaying equilibrium symptoms. That has not improved at all. She has some pain in her neck but, will take a Tylenol and lay down. This helps. Ms. Messing was seen first by a resident then by Dr. Ramsey. With each provider Ms. Messing had a written chronicle that she reports on. She report from the beginning to the current status of her injury. Her husband interrupts and interjects off topic things then she starts over so the appointment was lengthy. The main reason for the appointment today was to determine if the increase in her thyroid medication needs to continue and if it is related to the auto accident. Dr. Ramsey said yes that an accident or trauma can change the need for the thyroid medications. She will have a blood
