Beginning in 2004 I have worked primarily with diabetic patients within an outpatient health care system. However, the diabetes care and education provided did not occur in a vacuum, diabetes lends itself to numerous comorbidities that require coordination of care beyond that which is provided by one single provider.

Within my experience the roles have been quite clear in regards to who will provide each component of care and when referrals are needed and to whom to refer.
What is the structure and function of the team?
While no specific structure has been formally identified a multidisciplinary team leader approach has been used and proven effective. The collaboration with other providers such as cardiology, optometry/ophthalmology and podiatry has ensured bench marks and care standards are not just merely checked off but care being provided and with follow-up as needed.
How are desired outcomes established? How are they measured? Are there benchmarks for evaluating?
Within my work environment we follow the American Diabetes Association, Standards of Care coupled with Medicare/Medicaid recommendations in establishing benchmark/quality care measure. Each year these measures are noted for updates and with a check list built into the patient chart, as the appropriate ordering of testing and referrals are initiated and completed documentation is completed.
