On February 27, 2014, the Department of Human Services (DHS) Office of Quality Control (QC) identified an over issuance of SNAP benefits to Digna Carranza (Ms. Carranza). On March 5, 2014, the DHS Office of Program, Review, Monitoring and Investigation (OPRMI) received an allegation to investigate for possible Intentional Program Violation (IPV).
On January 31, 2013, Ms. Carranza made an office visit and completed a recertification application for herself and her two children. SNAP Benefits were approved and effective from March 2013 through February 2014. During the recertification process, Ms. Carranza provided proof of income for her employer.
