Symptoms/behaviors observed/reported during this service: The client informed the WYP he feels unhappy about staying at home all day, but fears to go out in the public, due to his anxiety from probation which causes irritable outbursts.

Interventions provided during this service: Intensive home based services were provided. WYP acknowledged the client's anxiety of probation. WYP assisted the client to decrease his irritable outbursts by offering to take the client into the local community to engage in some positive activity. WYP engaged the client in some positive activity in the home (playing cards) to ease the client anxiety to decrease his irritable outbursts. The client engaged in playing cards with the WYP and was able to keep his mind off of probation. The client worked with the WYP to look at employment online while noting down specific employment the client preferred. The client did not task without complaining and informed WYP he is doing more chores, because he is staying home more often. The client informed the WYP he is using some of the coping skills and he is seeing his therapist every Thursday to get treatment for his anxiety and depression.

Overall progress noted as of this service: Client needs to engage in a positive activity from 1x to 2x a week. Client will continue to be complaint in the home from 2x to 4x a week. Client will use his coping tools/skills to decrease his irritable outbursts from 4x to 2x a week. Client will continue to be more independent and relaying less on his mother from 2x to 4 x week..

Plan for follow-up and/or next intervention scheduled: WYP will follow up with the client's employment process, use of coping skills to decrease irritable outbursts, and WYP will enroll the client in a gym by the end of August as a positive activity. WYP will see the client on August 30th,
