The HIV care system is complicated and daunting for those that enter into its sphere. Medical and psychological care for those diagnosed with HIV/AIDs remains grossly under developed creating unique challenges for case managers as they work to address the difficult needs of their clients. Case managers are in a critical position to help assist with this complex system for a vulnerable population. The following paper will explore the use of the integrated case management (ICM) model at the micro level of advocacy, this approach will be demonstrated through the relationship between case managers and client’s navigating the HIV care system. Further challenges of ICM and possible implications for the future of case management will be analyzed. …show more content…
Integrated case management (ICM) models use an interdisciplinary approach with experts in multiple disciplines working together to develop individualized care plan across the life continuum. Authors Kathol, Perez & Cohen (2010) have worked to produce an ICM manual, which has been under development since 2006 but has been based on many years of research from multiple sources. ICM was formally introduced in 2008 at the Case Management Society of America (CMSA) and since this time over 1000 case managers have received formal training (Kathol R. , 2015). The evolution of ICM formed to address the needs of clients who were essentially falling through the cracks. Where their unique needs or illnesses border on the edge of non-communicating segments of medical and behavioural health sectors (Kathol, Perez, & Cohen, 2010). For each client there is an identified lead case manager who coordinates the necessary care and referrals, assisting them in navigating difficult systems in social and health care spheres. The lead case managers must hold shared decision making with the client and all other contributors and organizations involved or invested in the client’s wellbeing (Department for Communities and Social Inclusion, 2013). The individuals that make up this team must also have a willingness to learn and expand their knowledge into diverse disciplines, ultimately working together as educational and supportive units (Kathol, Perez, & Cohen, 2010). Supervisors must develop comprehensive training programs so that professionals will be able to address both medical and mental health barriers for their clients. These case managers must maintain a close working relationship with either an allocated physician or physician team, where they will conduct regular case reviews in order to ensure the appropriate interventions are introduced (Department for Communities and Social Inclusion, 2013). This also includes upholding a reliable relationship with a psychiatrist or psychiatric liaisons who specialize in various mental health conditions.
