Integration of Care Review and Evaluation at MRZ
Jewish Family & Children's Service (JFCS) is a non-profit, organization located in Phoenix Arizona that offers mental health, physical health, and social services to children and adults in Maricopa County. The Michael R. Zent (MRZ), Integrated Care Clinic is one out of the four clinics owned and managed by JFCS. MRZ is a collocated reverse integrated care clinic that focuses on providing care to patients by treating the mind and the body. Although the goal of creating the integrated care program at MRZ is to provide services that encompass Triple Aim principles this goal has not been achieved due to lack of education on the principles of integrated care and Triple Aim:
Improving the patient experience of care (including quality and satisfaction) – MRZ integrated care program currently has 40 patients while the behavioral health program has 3600 patients. The low referral of patients to the PCP and the lack of coordination of care and patients follow up has made it challenging to improve the patient experience of care. The Integrated and Behavioral Health Teams will be educated on referrals and coordination with PCP, member engagement and follow up.
Improving the health of populations - Patients currently receiving behavioral health services at the clinic have declined to transfer their physical health services to MRZ and patient receiving physical health have declined behavioral health services. This has made it challenging to manage co-morbidity and improve population …show more content…
For example, the education that will be provided to a Therapist is different to the education that will be provided to a medical assistance etc. Staff education will be provided in a classroom format and through printed materials. Patient education will also be diversified based on cultural and educational background. All printed patient education will be provided at a six-grade level or lower in the language preference of the patient. Auxier, Farley, and Seifert (2011) reported integrative care allows the development of an integrated care plan, provision of real-time intervention and better access to care. The resolution of the challenges faces at MRZ will lead to several benefits for patients and staffs such as the development of an integrated service plan, increase in the coordination of care and better patient
