Depending on the professional, whether it is a therapist or counselor, the intake form seems to be a suitable size in questions in order to help learn more about the different aspects of the client’s personal life.
B. Are the questions reading level appropriate for the clients? Please explain your answer.
For the length of the intake form, there are several questions on the form that are wordy and can be difficult to understand for some clients that may not be as literate. For instance, there is a section in the intake form where it asks questions about health and social information, there is a table with mental/emotional feelings a client may be experiencing. I think that this intake form should be updated, with more short questions, such as more “yes” and “no” questions, and if the social worker needs elaboration, the worker can ask during a first interview with their client.
