While searching for an intake form to critique, the Fairview Counseling Centers Adult Intake Form stood out among many others that were viewed. Choosing an appropriate intake form is the first step in gathering the necessary foundation to build upon. Furthermore, collecting data from the intended client enables the social worker to have a starting point in order to move forward in a direction. This direction can lead to further developing the client / social worker relationship or it could mean a referral to other agencies. In order to fill out the medication section, you first have to turn the form in another direction in order to fill it out. Although the form recommends that an additional sheet can be added to the medication list area, it is not provided within the form itself. The page that requires the client to fill out their symptoms gives examples as to what information they are looking for but does not do this for all questions in this section. For example, the form asks if the client has racing thoughts. Therefore, without clarifying what racing thoughts are, it will be a difficult question to
