The NFL Player Insurance Plan (the “Plan”) is a member plan of the National Football League Professional Football Players Insurance Trust (the “Trust”). There are two members to this trust, the Plan and the NFL Former Player Life Improvement Plan. Participants should refer to the Plan document, as amended, and Trust Agreement for more complete information. The League Office of the National Football League (the “NFL League Office”) was established by the Member Clubs of the National Football League (“NFL”) to coordinate and handle certain administrative matters and transactions common to all the Member Clubs, including the administration of the Plan. Eligibility requirements vary by benefit. For comprehensive medical, dental, prescription and EAP benefits, eligible players are listed on a Member Club’s roster in a qualifying benefit status for at least one game of a NFL playing season. A player’s eligible dependents also receive coverage. Benefits for active players begin September 1 for players included on a Member Club’s roster in a qualifying benefit status. Benefits for all other active players begin on the game day the player is on a Member Club’s roster in a qualifying benefit status. Eligible retired players are also covered along with their dependents and beneficiaries. A retired player represents an individual who has permanently ceased playing professional football and who has earned a vested retirement benefit under the Bert Bell/Pete Rozelle NFL Player Retirement Plan, based solely on credited seasons.
