I didn’t see the purpose of certain questions and didn’t feel we need to get that particular information. Or their were sections that things need to be added. For example my client was a widow but the martial status only had single, married, and separated. Also there was a section that talked about food stamps and WIC. I just fell that is something the case manager should ask and not the volunteer. After intake the volunteer group got together and talked about things we agreed and disagreed with the forms. We all seemed to agree that a lot of questions we had to asked seemed to be a case manager type questions. We all mentioned how at the agency we are not volunteers don’t get to do these type of forms. For example we all agreed that the release of information formed should be done with case manager as well. We also discussed in our group how a few of our patients decided they didn’t want to fill out the release of information form. Do they have to? What do we do if they don’t want a release of information? We all agreed that they don’t have to if they don’t want to. We have made a new intake with which we determined with forms should stay and which questions/forms should go. We will be turning that in on Tuesday
