On 10/01/2015 SC met with Pa in his apartment to conduct initial CMI assessment. Pa lives in a studio apparent. Pa appeared properly groomed and dressed in clean clothes. Home was messy and had dirty dishes in the sink and on top the counter. Nevertheless all utilities appeared to all be in working order. Pa was sitting in his recliner watching TV when SC arrived. Pa’s appeared to be cognitively intact. As the interview gets underway Pa was polite and cooperative as the questions shift from introduction, demographic and emergency contact, medical provider and use of medical service to medical condition.
