Identifying data about the client system (individual, family, group, community or organization):
Though I have had many experiences with working with different client systems, the client system in which I will be focusing on for the sake of this practice example will be a group client system. The group is composed of 21 participants, whom ages vary from 16-24 years old. The participants are members of the Opportunity Youth Program. The program requires that the participants are not currently enrolled in an educational setting or currently employed. The participants must also fall within a low-income bracket, though they are not required to show documentation for this specific requirement. Furthermore, the members of this group all live in

Though the goal of the program is to have as many participants as possible enroll is an educational setting along with finding permanent employment by the end of the 14 weeks, my personal goal for the Program Director is to maximize participant work/educational readiness. I have a hard time believing participants will do well in an educational institution or in the workforce if they don't have the skills to succeed.
Action 1: Place the participants in a work setting of their choice. Here is where the participants will be putting into practice what they are learning.
Action 2: Mandatory Weekly Workshops: 1 Time per Week/5 hours total. The Core Services Include: Life Skills, Work/Job Readiness, Educational/Career Exploration.
Action 3: Weekly Worksite Assessments: This is in place to assure the participants are receiving the best possible supervision.
Action 4: Weekly one-to-one check-ins with the client: To monitor progress and provide individualized services to the participants.

What I am mean is that I am my participants ages vary for 16-24 and I myself am 22 years old therefore, developing a relationship was relivitetly easy as many of us have a lot of commonalities. Again another aspect of the age that I though really helped out was the fact that many of the participants looked up to me, they saw me not only as a program director but also a student and as someone who grew up in their neighborhood. In addition, because I am still a social work student I was unavailable and out of the office at times which might have been a negative factor in the relationship between the group members and myself. Lastly, those participants who built a stronger relationship with me were more likely to take advantage of opportunities and has more success than did those who waited for me to present options and

Explain your understanding of the rationale for this EBP model. My time as a case manager, in a harm reduction program for homeless veterans has allowed me to have weekly sessions with a client for over two months now. The client is an Army vet with a history of substance abuse secondary to Post-Traumatic Stress Disorder (PTSD). In my effort to asses the client’s mental health on a weekly basis, I took notice in how the Feedback Informed Intervention (FIT), provided a unique opportunity for me and my client. Boswell, Kraus, Miller, & Lambert (2013) says there 's a moment in time when the patient and provider can look through a window together which the therapist and patient can catch glimpses into a unique world-view (Boswell, Kraus, Miller,…

Combined Pharmacological and Behavioral Interventions is a method of treatment that centers on the shared usage of medication and behavior treatment. This combination has been shown to be quite effective in treating children with ADHD and has several advantages over medication alone or behavioral treatment alone. With the addition of medication, the behavioral component of treatment may be able to be scaled down, thereby reducing the amount of time parents and teachers need to spend on shaping behavior. The dose of medication can be reduced for children using a combined approach. Parents knowledgeable about the use of behavioral treatments can apply such treatments during times when the child is not taking medication (i.e., in the evenings…
