Role of Medical Coding in Improving Safety and Patient Outcomes was an article that discussed how medical coders can improve patient safety and reduce healthcare costs. Some medical coders would say improving patient’s outcomes in the physician’s office is not part of their job responsibilities, but little do they know it is one of the major ones because entering correct codes that identify complications of medical care is very important. By correctly coding a patient’s exam will help all parties involved to correctly identify the medical issue and cut down any medical complication that may occur while keeping costs down will help not only the patient but the healthcare system as a whole. Other coders may see their roles limited to just summiting

Alphabetic Index and Tabular List Memo "The Alphabetic Index and Tabular List contains all of the medical terms in the Tabular list classifications" (Valerius, Bayes, & Newby, Blochowaik, 2012, p. 115). Abstract - Insufficient, disjointed and un-interoperable health care systems are the major causes of iatrogenic injuries by the means of adverse events (AE). Consequently, detecting and analyzing such events reveal problems affecting not only health care providers (HCP) and patients, but also the whole community. In this research we aim to architect a conceptual data flow model that helps in optimizing patient’s safety by the means of a newly designed Unified Medical Coding Structure (UMCS) and Structured Evidence Based Medicine (SEBM). Using this model we can assist in supporting clinical decisions at the point of care, preventing AEs as much as possible and collecting data when any missing AE occurred to be analyzed and used in setting rules…

Medical terminology is for ALL of healthcare, not just doctors and nurses but billers, coders, medical assistants, front and back office staff. Basically, anyone you see working in a doctors office, clinic, hospital or nursing home have medical terminology as their "language". In coding, to quote Professor Ray, " Medical Terminology is PRICELESS in coding". The understanding of medical terminology helps in coding and billing to know the highest certainty and specificity of the disease/injury process and treatment associated with it. By knowing and having the ability to code and bill correctly through your knowledge of medical terminology insurance companies will reimburse and the entire office and staff benefit from it.…

Patients feel confident in hospitals with safe hospitals with low rates of infections and complications. I believe high quality care and is synonymous with two things: safety and patient satisfaction. Patient satisfaction stems from our unit and hospital reflecting the six core values. Safety is the aim and philosophy needed to achieve the vision. Research has shown that electronic medical records improve communication, lead to earlier notifications and response times for adverse labs and findings, and better continuity of care for patients throughout the health care spectrum.…

Proper coding if very important because if a wrong code is used it can cause several problems not only for the patient but for the providers. Proper coding is very important in so many areas of the billing, ensuring accurate payment for the physicians to create a valid record of the patient’s medical history (www.healthcarebusinesstech.com/medical-coding). Proper coding can help expedite the payment for service and can also determine if a claim is approved or denied. It also tells us what services the patient had done and if the CPT code is wrong it could not only affect the billing but the patient would have false information in there file which the patient was treated for (www.healthcarebusinesstech.com/medical-coding). Physicians today are under extreme demand to see to many patients in a day and take too little time off to recover for their own health. If physicians took more time off for their own health, quality of care, results and patient satisfaction would be better. Studies have been done on about five-hundred hospitals. Surveys that have been done as well have shown about forty percent claim they have been asked to take more patients they can safely handle in a months’ time. If a patient is brought to the doctor’s office or emergency department for chest pain, or any other possible serious conditions, doctors find themselves ordering unnecessary test because they do not have enough time to fully examine them.…

In the attempt to call the attention to the importance of improving the quality and health care outcomes, in 1999 the Institute of Medicine had submitted a report called To Err Is Human: Building a Safer Health System. Although more than ten years ago, this report stressed the need of a redesign in the process of the patient’s care, little progress in the improvement of quality and safety has been achieved (Clark, 2013). Even though there were some important initiatives in the implementation of quality and safety after the report, only in 2013 The Joint Commission made a significant contribution in order to accelerate the process and enforced quality and safety through standards such as National Patient Safety Goals and Core Measures of nursing…

Formal standards in healthcare have derived from the need to develop a standardized system for informal applications within hospitals (Laureate Education, 2010). For example; hospital pharmacies, medical billing and clinical staff required an application to accomplish patient care in an ever growing field of information technology. Standards in healthcare were created to make processes work more efficiently (Hammond, Cimino, 2006) and comprise a set of rules and definitions that specify exactly what something is. In healthcare, specifically a hospital environment, a formal standard is useful because it permits two or more disassociated departments such as clinicians and pharmacies along with the billing department to work together in a cooperative way.…
