Towering over the tiny five-year-old girl, the enormous principal was wielding a giant wooden paddle. The girl remembered her mother 's words. "You are strong." In the following years and on multiple occasions, the man told the mother that the daughter “would never amount to anything.” These harsh words echoed in my head along with my mother’s voice. Responding with strength and admiration, she said, “We will see. Your judgment cannot break my spirited daughter.” Both comments compelled me to persevere and prove that I am strong enough to accomplish my goals. Thirty-five years later, a chance encounter would test my integrity and morals. Having surgery, an elderly patient expressed his concerns. In neurosurgery patients with chronic back pain undergoing complex multilevel procedures (PLIF OR XLIF), PACU RN observations reflect that this patient population has the biggest diversity of postoperative recovery outcomes and experiences. During their anesthesia experience, these patients present a dynamic challenge. Preoperative complaints of chronic pain and tolerance to narcotics are commonplace; additionally, preoperative interventions influence patients’ postoperative outcomes along with intraoperative anesthesia decisions and techniques that increase the anesthesia care complexity. PACU RN observations find major variations in the patient population’s outcomes such as complaints about nausea, pain, and delirium. Through problem identification, development, implementation, and evaluation, a critical review of the literature allows me to appraise evidence-base practice; furthermore, I will synthesize current research to determine the best anesthesia care practices for this patient population. At the current hospital facility, a proposed retrospective study evaluates current anesthesia practices for the designated patient population. A best evidence-based practice and current practice comparison disseminate findings to improving this patient population in outcomes and anesthesia
