The IS4H Maturity Model is a progressive array of diverse technical requirements that operationalize the 30 aspects included in the four objectives of the IS4H strategic framework. Accordingly, ranking at each maturity level implies that information systems for health complete all the corresponding requirements. The five maturity levels are the next:

The process of assessment uses the plan-do-understand-act (PDUA) cycle from quality management to define the IS4H Maturity Model Life Cycle with the next steps (Figure 6):

a. Analyze Requirements for IS4H Assessment
b. Develop Assessment Plan

e. Prepare for assessment conduct
The full assessment (3) and assessment development (4) and Report Findings (5) use a specific tool and involve the next steps:
f. Prepare Participants
g. Examine Objective Evidence
h. Document Objective Evidence
i. Verify Objective Evidence
j. Validate Preliminary Findings
k. Generate assessment Results
Reporting findings deliver assessment results for discussion (6) and to formulate an action plan (7).
