Wellness Coach Teresa Cain asked Mr. Lebron how his day was going thus far. Mr. Lebron responded that “I am alive, what more can I say”. Wellness Coach brought over a chair therefore Mr. Lebron can sit. When discussing how he has been doing with his old goals Mr. Lebron sigh and said that he has been trying to get his bills in order and has cut down to 4 cigarettes a day. Wellness Coach asked if there is new goals he would like to work on. Mr. Lebron said there are no new goals but there was change in his current goals. Mr. Lebron expressed that he has money owed to him from a law suit that happen several years ago and wanted to see why he has never received his payment. Wellness Coach informed Mr. Lebron that a number to the law firm in which handled your case can be found and further looked into. Mr. Lebron appeared to be anxious and tired from the day. …show more content…
Lebron is in the contemplation stages of change of his goal. Throughout the half Mr. Lebron has demonstrated that he is able to live on his own and complete daily living skills. Mr. Lebron pride himself on keeping his apartment safe and hazard free. Mr. Lebron has the ability to travel on his own in the community. However, Mr. Lebron still has shown no progress when it comes to his psychiatric appointments and rental arears. Wellness Coach Teresa Cain coach Mr. Lebron on how it is very important to keep up with his bills and medical and mental health providers. That his health can be in jeopardy of decline due to the avoiding of getting treatment. Mr. Lebron responded that he knows the importance that he is trying to get his rent payments in order.
