Mr. Michael is 70yrs old that weights 185lb and height of 65 inches. He has hypertension, diarrhea, depression, glaucoma, and osteoporosis. Blood pressure goes up when you are active and down when you are resting or sleeping. Sudden, dangerously high, and life-threatening blood pressure is called hypertension. His blood pressure is 170/80mmhg. Several types of drugs are used to control hypertension. These medications may be used alone or combination with other drugs. ACE inhibitors block production of substances that constrict blood vessels. They also help to decrease the build up of water and salt in the blood and body tissues. Patient takes ACE inhibitor captopril (Capoten, Novo-Captopril) 12.5mg orally 2-3 times daily. But before administering the medication need to check the allergies, foods, preservatives, or dye because patient can develop allergies with ACE inhibitors than with any other drug for blood pressure and also tell the patient to take it same time everyday. Captopril should be taken 1 hour before eating or on empty stomach. Patient cannot drink alcohol because ACE inhibitors can increase the low blood pressure effect and the risk for dizziness or fainting. Patient should avoid salt substitutes because salt substitutes contain potassium and it will increase the potassium level in the blood. The desire response of the drug would be the decrease production of angiotensin, increase in Vasodilation of blood vessels, decrease of excess tissue water and salt, lower the blood pressure, and also will decrease the workload on the heart. But there are some side defects of this medication are dry and persistence cough, abdominal pain, constipation, diarrhea, skin itching, dizziness, drowsiness, sleep problems, loss of taste and nausea. But there are some adverse effects of this medication are fever and chills, hoarseness, swelling in the face, hands, or feet. Patient also has diarrhea too. …show more content…
So he takes antidiarrheal difenoxin with atropine 2 tablets after each unformed stool. But the patient can not take more than 8 tablets per day, and before giving this medication, I need to check a complete list of drugs that patient is taking, vital signs, listen to bowel sounds, abdomen distention and patient weight. The intended response of this will be decrease in patient’s diarrhea, and GI motility, reabsorption of fluid from bowels and decrease activity of bacteria in the body. But there are some adverse effects that we should not ignore. Constipation, feeling of fullness, confusion, dizziness and drowsiness. So patient needs to take drug as it is prescribed and never take double dose to prevent constipation and avoid driving because drug causes drowsiness. Before administering him this drug, check patient’s blood pressure, heart rate and mental status. So he takes citalopram (celexa) 20 mg orally once daily. This medication will correct the depression and decrease the symptoms of depression. The most some side effects of this medication are dizziness, fatigue, and insomnia and also have some adverse effects headache, insomnia, anxiety drowsiness and hallucinations. Prescriber may start with a low drug dose and gradually increase it. Also should teach him that medicine will take 1 to 8 weeks before symptoms of depression improve.
