Our office contacted you on 2-8-17 letting you know your 2017 bar card was returned back to our office from our vendor. We believe we do not have your current contact information on file. After finding you online I wanted to reach out to you this way to see if we can get your updated Change of Address form. I’ve provide the form for you today. Once we receive your form we will re mail your bar card to the correct address. At anytime your contact information changes you will need to let us know.
