Hospital readmission rate is a topic that I’ve always wanted to know more about. Working as a case manager, readmission rates and bed days are important factors of the quality of care of the patients., (Axon, R., & Williams, M. 2012). Centers for Medicare and Medicaid will also reduce payments to hospitals with high readmission rates within 30 days, for Pneumonia, Myocardial Infarction and Heart failure. Also, in improving bed days, some physicians can overlook the safe transition of care of the patient during the discharge process. The most common readmission that I notice, in my current setting are patients with heart failure. This diagnosis is one of the leading cause of hospitalization and increased cost in the healthcare system.
