The impetus of this revision was the request from Temple T3 Transport team for a Curos protector cap on the port of a patient who was being transferred out of the outpatient chemotherapy infusion center. Unknowing what a disinfecting port protector, initiated a literature review by clinical staff nurses Dawn Papeika, RN, BSN, OCN, and Debra Dombroski, RN, BSN, OCN. Dawn and Debra wanted to ensure that the best evidenced-based practices were being used when starting IV’s, accessing ports and giving IV fluids and medications.

Education was provided during staff meetings in which EBP of handwashing, aseptic technique, and proper antisepsis of skin and IV hubs was discussed. A representative from the Curos Company provide the staff with an in-service on proper use of disinfecting port protectors. A poster presentation was provided as a visual reminder of proper technique. Unit clutter was determined to be one of the factors that limited access to hand soap, alcohol pads and gloves. It was determined that each staff member’s computer on wheels (COW) would be equipped with hand soap in the side pocket and that a supply of gloves would be placed in container in back of “COW”. In addition, small containers were added to the top of the computer in order to hold alcohol pads. These containers were labeled with reminders to “scrub the hub” with a visual depiction to reinforce that “seconds count”. Compliance was measured by observation use of aseptic technique, handwashing before and after procedure, and cleansing and drying times for hubs and skin along with the use of disinfecting port caps. The adoption of an evidenced based standard of care will ensure best patient outcomes. Just as quality and safety are so closely related, so is the importance of providing knowledge and skill in order to provide safety and prevent

Interprofessional Infection Prevention Team was formed to review NICU CLABSI data in an effort to eliminate CLABSI. This team includes nurse leaders, clinical nurses, providers, hospital infection prevention nurse, and a pharmacist. The team reviewed the policy and procedures regarding central line management and provided education focused on best practices to reduce CLABSI. Despite reeducation and reinforcing central line management, evidence based procedure standards for insertion, fluid change, proper technique for obtaining blood cultures, and regularly scheduled dressing changes, CLABSIs still occurred.…

Before operating, when intubating, catheterising or positioning a patient, gloves should be worn if there is a chance in coming into contact with bodily fluids. If catheterising, an aseptic technique should be followed closely. Hands can be carriers of Meticillen resistant Staph bacteria and introducing them to an invasive device, such as a urinary catheter, can give the bacteria the opportunity it needs to infect the patient (Mangnall & Watterson, 2006). Hands must be decontaminated with soap and water and an apron should be adorned before carrying out such a procedure. During the procedure a sterile field should be maintained and afterwards the apron should be disposed of correctly into a yellow bin bag and hands must be decontaminated…

Hand Hygiene is considered “the most important measure for preventing the spread of pathogens in health- care settings” because it reduces the spread of pathogenic organisms to patients and health- care workers. Studies in the mid 1800s showed that hospital acquired nosocomial infections and diseases were spread by the hands of healthcare workers. In the 1980s, the first national guidelines for hand washing were published, and many other guidelines followed. The US public health service released a training video in 1961 that demonstrated recommended hand washing techniques for healthcare workers. The video recommended washing hands with soap and water for one to two minutes before and after coming in contact with a patient.…

The senior dental hygiene student then returned to her cubicle and put on her Personal Protective Equipment which included her isolation gown, bonnet, facemask, and safety glasses. She again returned to the sink to rewash her hands and then put on her rubber cleaning gloves. Afterwards, she started by cleaning out her suction lines for about two minutes, and then she checked her water bottle determining she had enough for at least one patient. Incorporating this writer’s proposed solution of using alcohol disinfectant caps (ADCs) to reduce central line-associated bloodstream infections (CLABSIs) or any new evidence-based practice (EBP) into an organization is a multi-step process that includes: planning, engaging, implementing, and evaluating (Spring, Ferguson, Pender & Starin, 2011). This paper will focus on creating an ADC implementation plan, including methods used to obtain approval and procure proposal support; a background on CLABSIs; and the rationale and utilization of ADCs as a preventative measure with supporting literary evidence. It will also address the logistics and essential resources required for implementation. Everett Roger’s diffusion of innovation theory incorporates…

I feel that this is an experience that every student in healthcare should go through. One of the most important observations that I made concerns the various procedures of preventing infections in the operating room. The prevention majorly focuses on the patient, especially when considered the patient has open sounds during the operating process. At times, the practitioners tend to forget the importance of cleaning their hands with soap and water or with alcohol-based cleaners when interacting with the environment (Scott, Earl, Leaper, Massey, Mewburn, Williams, 1999). Notably, the assumption that the practitioner does not need to clean hands simply because they have gloves is misplaced, and it is often overlooked (Weber, Anderson, &Rutala, 2013; Karki& Cheng, 201)).…

An infection develops if the chain of infection remains intact (Potter et al., 2014, p. 166). In patient care it is important to use infection control measures to break the chain of transmission (Potter et al., 2014, p. 166). The practice standards displays that a nurse can break this chain by adhering to appropriate hand hygiene protocols and taking all essential measures to prevent the transmission of infection from the nurse to client or other health care providers (CNO, 2009). Hospital acquired infections are particularly challenging because many of these infections have developed antibiotic resistance (Pollack, 2010; Rosenberg, 2015). However, most hospital acquired infections are preventable through a variety of techniques, including the use of hand washing, equipment sterilization, use of gloves, sanitizing surfaces, among other things (Mehta et al., 2014; Monistrol, Calbo, Riera, Nicolas, Font, Freixas, & Garau, 2012; U.S Department of Health and Human Services, 2015). The key here is consistency, and a hospital culture committed to reducing and even eliminating hospital acquired…
