A review of her medical records indicates that she was recently evaluated by hospice, but was found to have a prognosis of 6 months or greater. She suffers from co-morbidities of NIDDM- stable, hyperlipidemia-stable, hypothyroidism-stable and HTN-stable.
At today’s visit she is accompanied by her sister. She is awake, alert but disoriented. She is a poor historian due to her dementia.
