SC completed a home visit with Pa on 02/9/2016. Present at the visit were the Pa DCW Maria, her husband Scott, and Neal Van a mortgage default broker from Financial Service for America a mortgage refinancing company. The Pa looked subdued, but reported that she was doing ok when the SC inquired. The Pa home is unhygienic and disorganized. The Pa reported that her water heater is not working and she just bought a second water heater in three months but has no one to install it for her. The SC informed SC DCW to call the SHARP and provided her with the number and a pamphlet. Nevertheless the Pa has working electric and reported that for heat she uses five space heaters because the gas I not working. The SC informed the Pa and the DCW of the dangers …show more content…
The SC asked Pa about her appetite and she reported that it is not very good but she drinks Ensures sometimes. The Pa was appropriately dressed and groomed sitting on the side of her hospital bed. The SC notices no changes in her appearance since the last visit. The SC reviewed PA’s ISP. Pa denied any other changes to her ADLs/IADLs, mobility, cognitive, social, emotional, or financial status. SC reviewed Pa 's ISP. Pa confirms that she continues to receive the service plan in ISP in the following type, scope, amount, frequency and duration: as specified in the ISP. Pa receives Agency Model PAS via Blessing4ever Home Care. The DCW provides supervision to the Pa 's to ensure she is carrying out the duties as it is outlined in ISP. PAS will supervise set- up and assist Pa with bathing, dressing, grooming, meal prep, cleaning, coordinating medical appointments and transportation 4 hrs. X 7 days. Pa confirmed that this is the amount of support necessary to ensure her health and safety and to meet her needs and goals; Pa reported satisfaction with her current services. SC and the Pa discussed self-directed services. Pa declined and reported that she is satisfied with the agency model and does not wish to make
