On November 9, I visited six clients with the home health care nurse Kim. Kim is a home health nurse who has been in the nursing field for the past 26 years and in the home health care nursing for about 6 years. One of the hard things about home healthcare for me was not knowing what to expect when I first walked into a client home yet, I hard a great teaching and learning experience. When we first meet in the morning we sit down and went over the schedule Kim has made on what client we will visit at what time and then we jot down main things we needed to do for each client as we visit them. The other service Kim provided to him was diet and exercise education, the importance of consuming protein for healing of his incision and fast recovery also importance of exercise throughout the day just walking around the house and so on. Mr. J. was on insulin because of the steroids medication he was on for his COPD; Kim educated him on how to use his pre-filled insulin pen, how to correctly check his blood glucose and the sign and symptoms of low blood glucose and what kind of food to eat when that happens. Since Mr. J was instructed by his doctor not to drive for the 6 weeks after his surgery, his sons or his neighbor picks up his medication for him from the local pharmacy. Mr. J’s support systems are his two sons and his neighbor. Yes, there are qualifications that an individual must meet for home health care. The qualifying requirements are assessed while the patient is in the hospital; some of the requirements include coverage of insurance, patient’s health status, patient’s support system and patient’s willingness to have a home health care. Due to his surgery Mr. J is not able to leave the home for his care by himself he needs other people help. The team Kim works with work together to make sure their patient success by charting and by calling each other at the end of the day to update each patient status with each other, also by constantly educating the patient and family, by setting up appointment for them with their doctors also helping the client find a way of transportation to get to their
