The one way that Home Health Care can be compare to be utilized is by answering patient any questions that they may have and educating them in doing good choices. Improving ambulation, the transporting of a patient, managing pain the interfering with the patient daily activity and improving the status of patient wound care.
The differences in between process care measures and outcomes care measures are process care measures will show how often home health care agencies have given recommendation on any care and treatment and there has been good research in patient results and the outcome of any care measures has shown results for any care that has been given to a home health facility.

The caregiver will talk and record the information that is provided to the patient, once they have gathered all the information the home health agencies will submits to Outcome and Assessment Information Set (OASIS) that will monitor and improve the outcome of the agencies.
A risk adjustment is a tool that is used to correct a report in a patient outcomes and by adjusting the differences among a specific patient risk, this will also make it possible for a hospital and physician to compare the performance fairly. This will also observe the improvement in rates for home health facility and the differences in facility.
When it comes to Medicare Part A / or Medicare Part B it will cover patients that are eligible for home health care services and the services that are provided are, skilled nursing care, continued occupation services, speech language pathology services, physical therapy and there are other services that are provided. Home health care facility services are coordinates by the patient doctor and the services the doctor is ordering.
The meaning to HHABN stands for Home Health Change of Care Notice and Advance Beneficiaries Non-coverage. HHABN is utilized by what is and isn’t covered by Medicare and it will explain in how the patient can keep on receiving services and

Home Care Associates Services that are provided are checking vitals, wound care, medication, communications with physicians. The Home health care will provide care in accompanying the patient to their appointments, bathing and grooming, rehabilitation, preparing meals, and light housekeeping (Home Care Associates: Philadelphia Area Home Health Care).

The quality of health care for a patient are different than other facility’s. The facility’s that I have chosen are not that different, what makes it different is the facility and services that each facility provides and their daily activities and health care for some patients that makes it different. The measures are based on the patient information that are provided by home health Care and compared to the state and national averages and then they are measured in health care
