On 1/8/2016 SC met with Pa and PAS/son in Pa's home for home visit. Pa appeared properly groomed and dressed in street clothes. The home was unclean, and cluttered with urine odor nevertheless, all utilities appeared to all be in working order. Pa reported no, falls, hospitalization, change in health status, or medication. SC reviewed ISP with Pa. Pa's goals remain unchanged on the other hand his needs has increased due to changes in he's health. Pa reports that services are meeting his needs. Pa is not requesting an increase in his care plan. Pa declined changes in his health status and mobility, ADLs and IADLs. Pa remains cognitively intact, very socially, and emotionally stable. No change in financial status was reported. In home support services is provided by Helping Hand Care.
