Although in home healthcare care patients receive services in the comfort of their home, there must be a structured evaluation. This structured evaluation comprises of these three phases which include nursing diagnosis, planning, and implementation.

The home inspection is also an important part of the assessment because it makes you aware of any unsafe conditions that increase the risk for falls, fires or infections. By doing the home inspection, you can assist the patients by directing them to services that may assist them to make their living conditions safe. It is vital that a nurse has a thorough assessment to make a precise nursing diagnosis for the patient.
To establish a nursing diagnosis, I retain my information from the assessment phase. This is also where my nursing judgment comes in to play. I often based the patient’s potential needs based on the priority of their diagnosis. Priority setting involves ranking nursing diagnoses in order of significance. With prioritizing, the nurse can attend to the client's ongoing needs and initiate the planning stage for the client.
During the planning stage, nurses use their critical thinking and problem-solving skills. In my experiences, I often utilize these skills to complete a plan of care for the patient. In turn, it assisted me to set interventions, expected goals, and outcomes for the patient. In home
