Grando explored the use of Orem’s Self-Care Deficit Nursing Theory practice model and applied it to PMHNs in advanced practice. Grando makes the connection between Orem’s beliefs about nursing and the theories of Nightingale and Henderson but she differentiates Orem’s focus on achieving health by addressing patients’ ability to engage in self-care. Orem identified the four metaparadigm concepts person, environment, health and nursing. Nursing actions according to Orem and Grando, fall into the categories of wholly compensatory, partly compensatory, or supportive educative systems (Masters, 2012, p. 184, Orem, 1985).

One of her core assumptions is that “self- care is the proper object of psychiatric mental; health nursing (Grando, 2 , p.6).Grando goes on to identify how the model is used to identify patients’ therapeutic self-care demands and assist patients to establish self-care systems, recognizing that many patients enter mental health care when their “established self-care systems are inadequate to cope with a stressful event and they are unable to develop new ones.” (Granado, p.6). The goal of PHN/PMHNP nursing is to find solutions to self-care deficits. Orem’s theory and Grando’s model was a good fit for the article. Grando connects the development of her model clearly and provides examples demonstrating the identification of self-care deficits, the patient’s basic conditioning factors, determining therapeutic self-care requisites, and how to develop self-care actions to meet the requisites. Clear examples of how to use the model in practice included detailing how she worked with a young adult, who was depressed and unable to get up in the morning to get to work.
