A review of his medical record indicates he has not has any significant health events such as hospitalization or falls since his last visit.
At today’s visit he is found in his room. He is awake and alert. He reports chronic pain which he describes as pain all over. He states that he has pain in all his joints. Which is worse when the weather is cold. He describes his pain as dull with a severity of 5/10. He states that his current pain regimen helps his pain but doe not completely take away his pain. He reports a fair appetite. He denies shortness of breath and distress. He ambulates without assistance with an unsteady gait. His comorbidities of chronic COPD, chronic HTN, chronic anxiety and chronic GERD are stable.
