D- Client arrived on time with her child, Fiona, for her scheduled appt. with this writer. During the course of the session, this writer and the client discussed a recent fax sent by the client’s PCP about the client hx Hep.C. According to the client, she did bloodwork with her PCP and is waiting for the results sometime this week. In addition, the client stated that she also did bloodwork for HOI medical and the tested negative for Hep. C, but needs to get the Hep. B vaccination. Addressing the status of her mental health services-the client admits to not contacting any of the listings of the mental health provider that was provided by this writer during last week session.
