A 61 year old Caucasian male brought to Henry Ford Main by EMS. The writer met with the consumer by his bedside. The consumer is aware of present time and the name of the president. The consumer was calm however unable to answer some questions on the assessment. However, consumer was rambling, delusional and disorganized thought, paranoid and unable to stay focus during his assessment. The writer had to redirect the consumer several times. The consumer denied any suicidal/homicidal ideation, no auditory/visual hallucination and no insight into the need for treatment. The consumer reports that” he slap his mother three time and was asking about his clothes and wanted to know why does he have a tether on his leg because he is not in jail”. The
