PHRs can also centralize the patients’ own health priorities, like tracking their food intake, exercise, and measuring their blood pressure. If the patient wants to, they can link their PHR with their personal physicians EHR whenever they want to. OCSCAR is a computerized national database, which main purpose is to assemble all of the data elements collected by surveyors during the inspection survey, that is conducted at specific nursing facilities. The data is collected from nursing facilities, for the purpose of certification for participation in the Medicare and Medicaid programs.
