Health care reform has been underway in the United States since President Obama signed the Affordable Care Act in 2010 (HHS). As the number of United States citizens with health coverage increases, insurers and regulator bodies are tightening their expectations on quality and safety. The Joint Commission is one of the regulatory organizations that accredits and certifies more than 20,500 health care organizations in the United States. The Joint Commission, an independent, not-for-profit organization, published that by mid-2015, 474 Sentinel events have been reported. In 2014, 764 events were reported. Sentinel events are occurrences that indicate unsafe processes, for example, wrong site surgeries, delays in treatment and retention of foreign body (object left in patient’s body after surgery).

Simply, the strategy of decreasing cost and bad outcomes works when changing the culture is also a focus. By creating a support structure that includes updating policies, leadership roles and communication pathways, health care professionals have been able to improve their safety culture (Groves, Meisenbach and Scott-Cawiezell 2011). However, health care leaders may be narrowing their scope by just focusing on improving outcome measures, instead of understanding the relationship between outcomes and the current culture. According to a survey conducted by the American College of Healthcare Executives, the number one concern of health care CEOs is financial challenges.
