According to Canadian institute of health information “Advance care planning or advance directives are associated with better patient experience and lower costs for the health system (2016, p.11). Health care providers hold the accountability to educate and inform the patient to scrutinize treatment options and to help them to make preferences by “engaging patient and families to reflect on their values, understand their disease process and progression, and develop a palliative care plan” (Dahlin, Coyne and Ferrell, 2016, p. 183).
I based this assignment on Mrs D., who is an 82-year-old lady and suffers with Acute Lymphoblastic Leukemia which is a is a cancer that starts from the early version of white blood cells called lymphocytes in the bone marrow. Leukemia cells usually invade the blood fairly quickly. They can then spread to other parts of the body, including the lymph nodes, liver, spleen, central nervous system (brain and spinal cord), and testicles (American Cancer Society, 2016). She was on chemotherapy after discussion with patient and family. Mrs D. expressed her feelings to staff by saying that she does not want to “suffer more” and “leave peacefully”. According to Elisabeth Kubler-Ross (1969) she was in “acceptance” phase of her illness (Elisabeth Kubler-Ross 1969). Mrs. D developed complications after receiving chemotherapy. Nursing staff consulted primairy physician and received oredereds for pain management, antibiotics and antipyriatics.
