Clinical service models are heterogeneous, as are patients, who have multiple chronic conditions, functional limitations, social vulnerabilities, and high mortality (Peter & Leff, 2014). The information gained through the interview will allow me to work with members of my community to help stimulate the usage of best practice to enhance healthy living. Home health nurses focus on promoting patient safety and autonomy in the home, as well at focusing on keeping patients in the home and out of the hospital. One of the practices that I will approach differently is focusing on the importance of medication compliance in the home to avoid re-hospitalization. Adherence to therapies is a primary determinant of treatment success. Medication non- compliance is a serious problem, which not only affects the patient but the health care system as well. Medication non-adherence in patients leads to substantial worsening of disease, death and increased health care costs (Beena & Jimmy, 2011). Another practice that I will change is wound care and assessment to prevent further complications or re-hospitalizations. Nurses must take into account comorbidities, individual wound-care requirements; assistance the patient may need due to physical or mental deficits, and nutritional support ("Providing wound care in the home: An option to explore," 2012).
