Health Care Integrator Observation

Health Care Integrator (HCI) met with Jose and at his school for the first visit of the month in order to provide services and be able to coordinate and individualize services that are caters to their needs and preferences of the family. HCI engaged in a conversation with Jose and his Paraprofessional Ms. Pagan and discussed Jose needs and preferences. Paraprofessional stated that Jose is a well behave kid and he is doing well at school. According to Ms. Pagan, Jose follows directive and no teachers have any complaint about him. HCI observed how Jose was interacting with his classmate and other school staff. HCI will continue to meet Jose’s needs and preferences.
