Health Care Integrator (HCI) met with Jose at his school and accompanied him to case address. Jose has a diagnosis of Disruptive Behavior Disorder NOS, Mood Disorder NOS, and Impulsive Control Disorder NOS, R/O ADHD. WSP services for Jose are Skill Building, Special Needs Community Advocacy, Prevocational Services, and Planned Respite. The purpose of the visit was to coordinate B2H services as well as to obtain on any current needs and concerns. HCI inquired if he contact or met with his Waiver Service Provide. HCI inquired about how Jose is doing at school. HCI commended Jose for remaining focus and getting the work done. HCI inquired about how Jose is doing at home and in the community. HCI asked Jose how his driving lesson is working out.
