The goal of therapy is to cure HCV infection in order to preventits complications as hepatic cirrhosis, decompensation of cirrhosis, HCC, severe extrahepatic manifestations and even death . The endpoint of therapy is an SVR , that is defined by clearance of HCV RNA in blood (detected by a sensitive assay) at 12 weeks (SVR12) and/or 24 weeks (SVR24) after the end of treatment course . In patients with advanced fibrosis and cirrhosis, eradication of HCV decreases the rate of decompensation and accordingly , will decrease but not abolish the risk of HCC. so, these patients should be contiously screened for HCC .
