Management of chronic diseases has garnered many concerns at Haywood Memorial, a 250 bed, non-profit health care organization, located in Houston Texas. Moreover, recent data analysis has revealed that the organization has a 45 percent readmission rate in which the majority of patients have a documented current diagnosis of diabetes mellitus (DM). DM is a chronic metabolic condition that affects the manner in which the body produces adequate amounts of insulin leading to elevated levels of glucose in the blood. When it is untreated or uncontrolled it can lead to stroke, kidney damage, heart attack and death. Houstonians who receive care at the facility have to drive a minimum of 40 minutes due to their residing counties lacking local diabetes care specialists. In addition, other noted challenges are lack of transportation which forces patients to drive for health care as well as an appropriate understanding of their disease state. Thus, Haywood Memorial is seeking a solution to decrease its readmission rate of patients with chronic diseases especially those with DM due to inadequate care post
