D-The patient requested to see this writer and expressed that she was unable to do a walk-in to the Hartford Behavioral Health as the walk ins is scheduled on Tuesdays and Wednesdays. The patient ensures this writer that she will do the walk-in tomorrow and will provide proof that she completed the intake process on Thursday. This writer stressed the concern factors of the patient illicit use of benzos as she tested positive on 06/17/2016, also with opiates. The patient admits to using pain killers from someone who given it to her as she missed her dose on 06/16/2016. This writer informed the patient about the risk factor of missing a dose as she will potentially put herself for a relapse, which evidently happened in this case. This writer
