The Ministry has created a guide to assist with a Harmonized Practice for Identifying and Linking Providers. This practice consists of four steps which are listed below and the Ministry document is attached for your review

Harmonized Practice - How to Identify and Link Duplicate Providers 1. Search for all duplicate provider records.

2. Review all records and identify the winning provider record. A winning record is the record to be used on a go-forward basis once all other duplicated records have been identified. See detailed instructions - Step 2: Determine the Winning Record.

3. Link the duplicate records. See detailed instructions – Step 3: How to Link Duplicate Providers.
