Espinoza stated that on the claimant’s LDW 11-25-2015, she was told by Supervisor Navarrete that he had information from the claimant showing that he had a pre-existing industrial back injury. When Ms. Espinoza questioned Mr. Navarrete about the information he allegedly received from the claimant, Mr. Navarrete told her that on 11-25-2015, when the claimant had asked for a day off, he mentioned to him that he needed to see his doctor for an industrial related injury, which occurred when he was employed by a car dealership. The claimant did not specify the name of the car dealership where the injured occurred, but did state that he injured his spine and the discs in the middle of his back (Ms. Espinoza stated that our office needed to interview Mr. Navarrete in regards to the allegations made by the claimant). Ms. Espinoza stated that the claimant completed a safety training, which was presented to him on his DOH. Jorge Soriano. The applicant testified he developed pain in the afternoon on the same day of the injury he testified he is only experiencing pain to the lower back and his neck. The applicant claimed he did request treatment and had communicated such request to the insurance carrier, but the response he received from the doctor as well as the carrier was they needed to focus on the treatment for the right shoulder before they could address the back and neck injury. Today 11-17-15, a Zarp Journeyman Laborer was prepping an excavation at the north elevation on street level. The employee removed his gloves to use the engineer’s rod to check the elevation. With his gloves off checking the elevation he slid tis right hand along the perimeter edge of building plywood protection. On Friday 10/16/2015 at approximately 0230 hours, Florida Hospital Security Operation Command (SOC) radioed FHEO Security Officers in regards of a 44V- Enforcement Escort Visitor off Property. Security Officer Allan Topher (114) and Security Supervisor Steven Evans (407) responded, upon arrival we were informed by ER Nurse Mhel Jainga that discharged patient, James L.
