Hampton Home Reflection

Monday, October 5th: For the last clinical day at Hampton Homes, I was in the role of doing home visits. We made two homes visits today. Both home visits were to clients I had never visited before. One client talked a very long time about her community and environment. I asked her questions as we talked, and she revealed a lot about the area she lives in. This goes along with learning outcome four, dealing with how the environment influences the health of families, groups, and communities. The client mostly addressed the family dynamic of most families in the community and how it relates to safety. She said that the parents (typically young moms) in the area of Hampton Homes, let their children run all around the community by themselves. Oftentimes, …show more content…
There was a lot of dysfunction going on with my patient who had the infection of her stump. Her other problem of obesity complicated her situation and is extending her healing time. We have learned in previous nursing courses like medical surgical nursing that obesity and high fat content, makes the healing process from surgeries and infections take longer. This patient had a recent graft done which is to be used for her hemodialysis, and this will likely take longer to heal. I saw signs of this, and observed this when talking with her physician about her plan of care. We both examined her site and noted that it was reddish pink and irritated. Although he said he did not believe it to be infected at this time, it will take longer for her arm to heal than other …show more content…
I will teach another student how to preform a clinical skill/and or something new I didn’t know by the end of the day. I will help another student with one of their tasks by the end of the day. My patient goals were: My patients will be able to explain what they need to do upon discharge to maintain health. My patients will state ways they will prevent being hospitalized in the future. My patients will question what their medications are for, without being told right away by the time I say I have their

I am growing and developing better time management skills and feel more confident and less stressed with a heavier patient load. I really enjoyed the added responsibility and am gratefully for the opportunities to practice my skills this semester. Finally, #5 Personal Learning Objective: by the end of semester I will demonstrate effective patient advocacy by communicating the patients wishes and decisions regarding their treatment options to the physician. I used advocacy multiple times during these shifts to express both the patients desires and needs regarding the care to multiple members of the healthcare team. Kalaitzidis and Jewell (2015) insist that nurses at as a translator between the patient and healthcare team and this is one of the reason that…

Dawn was very happy with her mid-semester evaluation. She didn’t expect her supervisor at Day One to give her such high regard in all areas of the evaluation, such as a five, meaning, “greatly exceeds the standards”. However, there was one area of the evaluation that she only scored a four, meaning, “exceeds standards”. It was in regards to punctuality. This particular area was “arrives on time; gives ample notice if unable to attend”.…

Katelyn Baker 09-27-2014 Monday-Thursday 7:30-9:30 MRSA: Lori Popp’s Story I had the chance to listen to guest speaker Lori Popp talk about what happened to her after going through a bariatric operation and the complications that came with it. Her story really opened my eyes to what is happening in the world and health care settings. She taught me to be cautious because not every health care worker takes the proper precautions when working with their patients. This is how she acquired MRSA, because her surgeon did not follow proper protocol. Lori endured multiple antibiotic treatments, but more than anything she had to endure a terribly great number of surgeries; currently somewhere near 60 surgeries, to help control her MRSA infection because…

Hello everyone, I am Diane Porché; in the Clinical Mental Health course, been in Denver for 45 years; have a daughter who is 35, and a grandson who is nine that keeps my spirit alive. As far my clinical site, Salvation Army is a six month (compulsory live-in) alcohol and drug treatment facility, providing for men and women age 21 to 60. As far as fears, being in a room with a client alone, and assessing the client using the ASI, it's a little overwhelming. Furthermore, how to motivate the client in finding novel ways to manage their feelings, control impulses, work toward concrete goals for recovery; alone with confronting manipulation, teaching a relapse prevention course and maintaining my case load are further concerns.…

For reimbursement as well as to measure patient progress it is important for physical therapist to write goals including short term and long-term goals. My first short term goal was the “Pt will be able to sit on edge of mat with no assistance for 2 minutes 3/5 times in 3 weeks to help with toileting.” It is important for this patient to be able to be independent in sitting and one of the main tasks we do seated is toileting. By giving this patient independence in this task he will be able to get closer to staying at home by himself and returning to his prior level of function. The second goal I chose was the “Pt will increase AROM in L knee by 10 degrees in 3 weeks to help with ambulation and climbing stairs.”…

While I have not had nearly as many hardships in my life as Jeannette Walls in her book The Glass Castle, there are still some things that her and I have in common. Throughout the book, Walls endured many struggles while traveling with her dysfunctional family. Jeannette’s father is constantly getting in the way of her success, from stealing her money to not always making her go to school. Eventually, she leaves her parents behind and makes her way to New York City, where she goes to college and finds success. Similar to how Jeannette had to overcome the obstacles between her and her success, her parents in The Glass Castle, I had to overcome the obstacle between me and my success, my bones.…

They call them “Tap-taps,” the brightly painted open back trucks that transport passengers through the dusty streets of Haiti. A traveler taps the side of the vehicle to board, pays, rides and then taps to jump off at a desired destination. For one particular man who sat with his face covered by a towel, that destination was the Diegue clinic in Port-au-Prince. All I knew from intake was that this patient was here for a foot issue. I noticed the adult sitting uncomfortably, a towel wrapping his foot, as his eyes scanned around the room.…

I had set in mind the objectives that I wanted to accomplish for the day. One of the goals was to do a complete charting on one patient and my other goal was to work on time management. Charting in PACU is quite different than a regular floor because in PACU the charting has to be every five minutes times three when the patient arrives to the unit and then every fifteen minutes for about two hours until the patient is stable. When the patient is stable the vital sings and assessment are done every thirty minutes. As soon as the patient arrived to the floor, I was ready to connect my patient to the monitors and assess my patient.…

Good communication will help to prevent unwanted outcome(s). Therefore, learning how to communicate and collaborate with others will be beneficial especially for the patients. This has had a large impact on the families physical environment, and education. Due to the fact that the father has not been able to consistently hold a job, there has been many times where the family has run out of food in their home. A healthy physical environment includes having access to food. Healthy nutritious food has not always been easily accessible for the Montgomery children, and over time lack of nutritious diet will affect their health. Safety is what new home owners with kids look for. Corresponding with this, at first glance Tri-Cities Washington seems like a contained environment to raise youth in. As it is isolated from any big city and is a ninety minutes away from “major crime” areas such as Yakima. Under high magnification, a safety concern can be found in this area. It is ironic that such can be found in a town that has grown up around the Hanford sight and farming opportunities.…
