HSC Pediatric Center: A Case Study

The patient Navigation will serve as guide to the disadvantaged African Americans and Hispanics in question by identifying the specific healthcare needs of these patients such as inadequate or lack of insurance coverage and subsequently fulfill these patient’s needs thereby promoting better patient outcomes. In addition to the fulfilling the health insurance needs of these patients, the patient navigation tool can also serve a communication medium with other departments such as financial services, patient transport, and patient/family education, thereby enhancing the health of these patients and promoting well-being. The elimination of certain non-medical day to day stressors which are not intrinsically connected to a patient’s health may promote a faster patient recovery. A Patient/family that do not have to worry about the financial resources to address non-medical burdens such as rent or mortgage while admitted in the hospital for example may have a faster recovery than the one who is preoccupy with financial constraints. The patient’s navigation will also serve as interface between the patient’s admissions at HSC Pediatric Center and the patient’s transition back to home and the community. The lack of adequate discharges and planning can result in detrimental discharge consequences. An inadequate discharge procedure and a poor transition planning to home or community increase the risk of a readmission of the patient in question for the same problem.
