Greenville Rancheria Gym Guidelines
Appointment Agreement
I, _______________________________ am aware that the Greenville Rancheria gym is not a drop-in facility. I understand that my appointments in the gym are scheduled the same as any other appointment at this clinic and that I am expected to be on time. It is also clear to me that if I am late for my exercise class my time will be cut short. If I am more than 15 minutes late and did not call ahead or make previous arrangements I understand that I may not be allowed to exercise at all that day.
In the event I cannot keep my appointment I will give at least a 24 hour notice and if I don’t it will be considered a “no show” appointment.
