The patient arrived emotional and in tears as she learned that her mother is diagnosed with cancer. The patient plan to visit her mother in North Carolina next week as she plan to leave on a Wednesday. The writer explained to the patient about guest dosing as the patient is not eligible for take home bottles. Please note, the patient is aware that the writer will need to discuss her situation with the Practice Manager as the patient's past/current UA's are positive and the patient need to have a negative UA result for the guest dosing application. Should the patient be granted for the guest dosing, the writer will complete the application. The patient was made aware that every clinic outside of the clinic requirements are different and there may be a chance that the patient may not eligible due to her UA's result.

In the meantime, the patient denies any recent relapses since her UA result; however, her UA is negative for metabolites. The patient denies any falsification. No evidence of SI/HI.
