DRC MHSS ISP Quarterly Review 11/4/16-02/01/17 Grocery Skills During this quarter, Kenyatda had shown a bit of improvement with his diet and nutrition. Kenyatda manage to make wise food choice the majority of times. Kenyatda also improved on his budget. He would always make a list of the items that he needs from the grocery store.
Psychiatric Wellness
During this quarter, Kenyatda visited his primary care physician due to back pain. Kenyatda experienced back and it had been going for a long time now. The doctor recommended Kenyatda to practice exercise while home. Kenyatda still working on maintaining his the ability to exercise his back

Interpersonal skills
Interpersonal skills still is an area that Kenyatda needs to work

Kenyatda has improved a lot with his cleanness skills. Kenyatda cleans his living room, kitchen, bedroom, and bathroom. The plan is to continue to work with Kenyatda on maintaining a clean living environment. AA meetings
During this quarter, Kenyatda did not attend any AA or NA meeting. However, when Kenyatda was admitted to the chippenham hospital he stated that he attends few groups and they discussed about AA and NA meeting related.

Community integration
Kenyatda did not participated on community integration; however, there were few time that Kenyatda and MHSS went to downtown mall for a short walk in which Kenyatda enjoyed it.

Maintaining wellness
During this quarter, Kenyatda had a crisis that took on January 3rd of this year. Kenyatda was using drugs and alcohol constantly over weeks. On day Kenyatda came to PSR group and disclosed about his current situation. Kenyatda voluntarily to go to go to the UVA ER. Kenyatda called the police and informed that he was experiencing suicide thoughts. Kenyatda was taken to UVA ER the following day; kenyatda was transported to chippenham hospital. Kenyatda was admitted to the hospital for 5 days where he was treated. Kenyatda was released on the 10th of
