I am in receipt of and have reviewed your grievance appeal dated April 6, 2015, regarding your complaint. You claim Corrections Case Manager II (CCMII) Stacey Iburg refused to provide you with an Informal Resolution Request (IRR) form on several occasions. You are requesting an investigation into the matter.

The response you received at the Informal Resolution Request and Grievance level adequately addressed your complaint. CCMII Iburg refuted your allegations you were denied an IRR form. There is no evidence to support your allegations CCMII Iburg refused to issue you an IRR form. Your appeal is denied.
